The rules for participation

• member will charge no more than $100 the first 1/2 hour of consultation
• member will provide proof of professional liability insurance, ($100,000 per occurrence/ $300,000 aggregate) annually.

Billing is from July 1 to June 30, and new panelists will be prorated accordingly.  We will send you an invoice according to the following pricing schedule: $150 per panel, discount of $25 per panel for 4 or more panels. To enroll, submit the form below
